Health Care POA & Advanced Directives Summarization

Private labeled in app experience and client deliverables of incapacity planning document summaries that further advisor and client understanding of estate documents, with actionable insights.

1. How long does it take to process a request?
Turnaround is up to 3 business days.  Financial Power of Attorney reports include a  human in the loop process by Estate Specialists. 

2. What happens if a wrong or incomplete document is uploaded?

Errors are flagged during initial review. Users can edit submissions before the review begins. Service levels are based upon when a user resubmits.

3.  Who reviews the documents?
U.S.-based estate professionals, including backgrounds in law, planning and trust/estate administration. Our proprietary logic supports the process, but does not replace our Estate Specialists.

As the client's financial advisor, you should also review the documents you submit and the report in accordance with your policies and procedures. 

4. How do I get the most out of my request? 
Include as many documents as are applicable to your report request in the single report request.  

5. What documents are and are not supported? 

Generally, Medical Power of Attorneys, Health Care Power of Attorney, Health Care Surrogate, Living Wills, Advanced Directives, and Health Insurance Portability and Accountability Act (HIPAA) authorizations are supported.  

6. Can I upload a Health Care Directive signed by non-household members? 
No, these are considered third-party estate planning documents and are not designed to be ingested under other households. 

7. Can I include multiple household members as part of a single report request? 
No, Holistiplan Estate incapacity planning documents are summarized on an individual household member level. Please select the appropriate household member within the Health Care Directive module’s upload modal. 

8. Can Advisors customize the report?
Yes, Advisors can edit and hide system generated insights and add additional insights to the report. 

9. Are citations linked to report insights?

Not currently. Holistiplan is exploring hover-over highlights and advisor-facing “next step” reports.

10. What fiduciary roles does the tool identify?

  • Health Care Agents. 
  • Guardian Nominees. 
  • Authorized Representatives for HIPAA

11. What is identified in summaries?

  • Agent names and order of designation
  • Referenced Jurisdiction
  • Durable and Springing provisions
  • HIPAA release language
  • Agent powers, including, but not limited to: Anatomical gifting, Disposition of Remains (where applicable)
  • Advanced Directives, including desires regarding life sustaining treatment and to remain at home

12. Can draft documents be submitted?

Yes, but review output will be limited commensurate with what is available in the draft.

Special note: Please be aware that clients using state adopted statutory forms generally do not indicate their intention to grant agent’s specific powers on draft documents.

Are custom firm disclaimers available? 
Yes, firm administrators may set firm disclaimers in settings.
